Besides the relative dearth of villains in the Marvel Cinematic Universe, another big problem could be the fairly dainty amount of heroes flying around as well. Part of the reason the Civil War event was so huge originally is because it drew in literally every character Marvel had going and again, in over half a century, they've amassed quite a few characters. The main series had a cast of hundreds, if not thousands, and then there the countless spin-offs and tie-in issues which showed how everyone from Spider-Man to the Sub-Mariner were affected by the whole shebang. It also allowed for a pretty cool series after the whole thing had ended called Avengers: The Initiative, which saw the government franchising out superhero teams for each state in the country, with Iron Man overlooking the whole thing. Easy to do in the comics, when there's superheroes to spare; less easy in the films, where it's been established that metahumans have only just started showing up, and so far all of them are in the Avengers. Or else they've only appeared in Agents Of SHIELD, and most people don't watch Agents Of SHIELD.
