For the evil Doctor Strange who was shown during the final seconds of the Multiverse of Madness trailer, the mind immediately goes to the fourth episode of Disney+'s animated What If...? series.

Titled What If... Doctor Strange Lost His Heart Instead of His Hands?, this offering saw Christine Palmer a passenger in the car accident which, in the MCU, caused Stephen Strange to lose the use of his hands. In this alternative take on the Sorcerer Supreme's origin, he actually emerges relatively unscathed from this crash. Unfortunately, Christine isn't so lucky and dies from the injuries suffered in this wreck.

Following this tragedy, Strange heads to Kamar-Taj and learns his magical craft. Upon realising that the Eye of Agamotto can reverse time, his attempts to go back and try to save his beau all end in failure. With it revealed that the Ancient One has split Doc Strange in two - the evil Strange Supreme and the standard, heroic Doctor Strange - these pair of alternates do battle, with the villainous persona winning out.

With Strange Supreme finally having the power to save Christine, his 'one true' is disgusted by his appearance as reality itself is torn apart. Supreme saves he and Palmer in a small pocket of reality, but then she disintegrates and leaves him completely alone across all space and time.

Could some take on Strange Supreme be the evil Doctor Strange seen in Multiverse of Madness? That remains to be seen.
