Strangely enough, lower abdomen tattoos in prison - of eyes, rather than a mouth - tend to denote a homosexual prisoner. Given the suspicions that the Joker's affections for Batman do run into sexual waters, the fact that there's an enigmatic smile there could be a tongue-in-cheek nod to those theories.

Otherwise, it's probably just another opportunity to show off his branding.

3. HAHAHAHAHAHAHA

Again, it's probably something of a space filler, but the font and letter placement definitely calls back to the iconic The Killing Joke panels.

Beyond that, there's probably little in specific story terms: he's the Man Who Laughs, of course he'd be covered in ink like that.
