3. Water Temple - The Legend of Zelda: The Ocarina of Time
http://www.youtube.com/watch?v=dXHnhi84fQk I can already hear the groans coming; yes, the Water Temple is in many ways the bane of Ocarina of Time, but it's also one of the most impressively designed and cleverly challenging levels I've ever played. This level, appearing roughly half-way through the game, is perfectly placed in the sense that players will be totally familiar with the game's mechanics by this point, and may even have become a little complacent that they're making their way to the end a little too easily. Well fear not, this fiendishly difficult maze requires you to raise and lower various water levels throughout the temple to progress, resulting in plenty of headache-inducing highs and lows, as you tie yourself in knots trying to figure out the proper solution. Though I can't confess to playing the level very often due to its exhausting nature, the feeling of accomplishment once it's behind you is nearly unrivaled. Design-wise and with regard to its placement in the scope of the game, it's absolutely fantastic (though you might not want to admit it).
