SCP Containment Breach has a multitude of horrific monsters that could get on this list. But, when it comes to the creatures that are truly terrifying and that fill players with dread, you can't choose anything other than the main monster itself. SCP-173.
Akin to Doctor Who's Weeping Angels, SCP-173 forces you to look at it in order to survive, making encountering it incredibly tense. Most people upon seeing the creature, will simply close the door and run, a testament to just how dreaded it is.
On top of that, it appears at the most inconvenient of times. The monster doesn't have set spawn locations, so it tends to just appear whenever it wants, making the game an exercise in caution. Entire rooms become unusable due to the statue sitting in the corner.
And it just looks weird. Creepy weird.
