10 Most Grotesquely Violent Video Games Ever
8. Painkiller

Painkiller is a 2004 title originally released for the PC. The game sees you play as Daniel Garner who, along with his girlfriend, has died in a car crash. Daniels girlfriend is enjoying life (death) living it up in heaven but poor Daniel is stuck in Purgatory tasked with fighting off hordes of monsters in different settings. Each level takes place in a different area and the monsters you face are varied and fittingly terrifying every time. Some of these enemies include mental patients missing limbs, zombies and some extremely sinister clowns.
To help you commit your genocide of these hellbound monsters, the game gives you access to a multitude of extravagant weapons. These weapons include the stakegun which enables you to shoot stakes at your enemies and pin them to walls, electrodriver which shoots lightning at your opponents and the titular painkiller which is a saw type weapon used for melee. Painkiller has excessive amounts of blood and gore but what can you expect from a game inspired by classics like Doom and Duke Nukem?
