It makes no damn sense at all. Seriously, how is that platform even suspended there? But players are at least able to have a little fun by popping a cap in the NPC and causing him to tumble gloriously from the platform.
As YouTuber White-Half discovered during their experimentation, this NPC is actually bound to the replacement heart you're able to destroy in the hospital during the mission. If you destroy the heart, the NPC can no longer be shot, because the game has already registered them as dead.
