5. Jen - Prey

2006's cult classic FPS Prey drops one hell of a devastating mid-game plot twist, when protagonist Tommy's girlfriend Jen - who is kidnapped by aliens at the start - is revealed to have had her upper body surgically grafted onto the lower body of a reptilian creature.

More to the point, said creature has designs on killing the player, and so, you've got no choice but to engage the Jen-Monster-Thing in combat.

After defeating the creature in a boss fight, Tommy tries to figure out how he can save Jen, and as players, we might assume that Tommy could somehow use his spiritual powers to restore Jen's regular form.

But tragically, Jen is quick to tell Tommy that nothing can be done to save her life, before she asks him to put her out of her misery, and the player has to pull the trigger themselves.

Even if you might've hesitated to kill Jen, thinking there must be a way to bring her back, there isn't. 

At least given the series' supernatural nature, we know for sure that Jen ends up in the afterlife-like realm known as the Land of the Ancients.
