3. Crash Team Racing's North American Release Was Unfinished
In a gaming climate where online patches were nowhere to be seen, Naughty Dog accidentally released Crash Team Racing unfinished, with the NA release containing a broken Penta Penguin character who upon picking up an Uka-Uka power-up, actually summoned Aku-Aku.
A simple mistake, but in addition there were clips of a tester saying "Penguin Yay 1" and "Penguin Yay 2" - both soundbites that were meant to be removed from the final pressing before going to retail. These audio clips remain in the NA version, but were fixed for the later European release.
