Coming in at the start of the second game, exiting a hospital room, it shows returning players that killing the culprits in the first game only aided in making things worse for Max.

This visceral monologue highlights the desperation that grief and depression brings. It conjures up horrible thoughts of literal digging into a skull and scraping out the contents, really showing that Max will try and do anything to rid himself of his torturous past, even creating lies (denial) to temporarily ease everything.
