8. The Pegasus Project

Everyone loves a good conspiracy theory, and the Next Generation episode The Pegasus presented one of the series' best. Commander Riker's old vessel, The USS Pegasus, conducted a secret test of a 'phasing' cloaking device when things went catastrophically wrong.

Given that such a device was strictly prohibited by the Federation/Romulan peace treaty, it's understandable that it was classified and the men responsible charged. But no further information was ever offered up as to its origins.

We know that the device was created in secret by Starfleet and given to Captain Pressman to field test. However why this was green-lit, or what happened with the full inquest the episode promised, has never been discussed. Likewise, quite what impact this had with relations between Earth and Romulus is anybody's guess.
