2. The John Hurt Doctor's Actions In The Time War

It's not yet been firmly established what exactly the Doctor's role in the horrifyingly brutal Time War was (well, Clara finds out when she reads the book of Time Lord history in "Journey To The Center of the TARDIS", but she isn't telling), but the John Hurt character revealed in "Name of the Doctor" seems to be responsible for the majority of the atrocities.

The most likely explanation for the presence of John Hurt's Doctor (and the only one that doesn't completely make a hash of the established continuity) is that he is a future incarnation of the Doctor that contains all his hate, rage, and negative emotions... in other words, the Valeyard, or his 13th incarnation, brought back in time by the Time Lords to do their dirty work.

This theory is backed by the fact that it's already been shown in "The End Of Time" that the Time Lords had no compunction about using the Master to accomplish something similar, and they even gave him a full set of new regenerations for the trouble. Whatever destruction he was responsible for, the 11th Doctor can hardly contain his disgust for the man, and why not? His mere existence proves that the Doctor's goody-goody image is merely a facade that contains some truly dreadful things.
