4. The Visible Crew Members - The Last Of Us
Game of Thrones and House of the Dragon aren't the only HBO shows that have brought a few rather awkward mishaps to our screens over the last few years, with the recent The Last of Us adaptation also containing a pretty incredible mistake.
In Episode 6, Kin, as Ellie and Joel make their way through the snowy terrain during their search for the latter's brother, Tommy, a wide shot suddenly reveals some unintended cameos.
Though the pair appear to be on their own as they cross a large bridge, a fan on Twitter actually spotted that they were joined by what looked like a few crew members filming them on the ground.

Again, it's a tiny error and one that was swiftly edited out of the sequence (via CBR) once HBO learned of its presence in the tense river crossing scene. But it's remarkable to think that these crew members were able to make it all the way through the entire shooting and editing process without being spotted by anyone.
