In the final episode of Star Trek: The Next Generation, Q continues his trial of humanity by presenting his old sparring partner with an alternate future. It's actually fairly similar to the one shown in Picard, he's retired to run a vineyard, still keeps in contact with many of his former crewmates, and there's a strange anomaly that could cause the destruction of mankind.

In this potential future, Picard is suffering from an Alzheimer's-like condition called Irumodic Syndrome, which is seen as an explanation for his flashbacks. When Picard returns to the present, it was revealed that he did indeed have a structural defect in his brain that could lead to similar neurological conditions in his future.

This became a key part of Picard season 1, as Picard came to terms with his mortality and his failing body. In season 2, this has been remedied by a synthetic replacement, however, Q once again throws him into a nightmarish alternative future. Will he make it back this time?
