3. Carl Grimes - The Walking Dead
While everyone saw Glenn's death coming, nobody who calls themselves a fan of either the television series or the comics they're based on saw the death of Carl Grimes as anything more than a remote possibility. In the comics, Carl survives to the very end, so killing him off on the show seemed unlikely.
Then, without warning, the showrunners decided to do the unthinkable; they let Carl get bitten on the abdomen. In the world of The Walking Dead, a bite is always fatal (unless you can hack off the limb that was bitten), and sooner or later, he would succumb, die, and rise up again as a new member of the walking dead.
The thing about killing off Carl that's even more impactful is that the actor who played him, Chandler Riggs, was blindsided by the death even more than the audience was. He had just purchased a home in Georgia and was planning on remaining on the series for years, but the bite took it all away.
Carl's death may have been a surprise to most viewers, as well as Riggs, but the character did have a tearful goodbye with his friends and family. After saying goodbye, he went into a house and shot himself in the head off-screen.
