Pre-capture/torture, Theon displayed many of the qualities found in narcissists - grandiose sense of self-importance, expectation of favours and compliance, haughtiness and sense of entitlement - but now that side of Theon has been, er, cut out of him what can we say of his mental state? He's certainly not a well man.

Due to the trauma inflicted on him, Theon might have suffered a psychotic break. He shows signs of Dissociative identity disorder with comorbid PTSD and Stockholm Syndrome. The majority of patients with DID report past physical or sexual trauma, something we can safely say that Theon has endured.

It's difficult to say whether Reek is a totally separate personality to Theon as his insistence that he is Reek seems to be in spite of himself, and it is perhaps possible to see flashes of Theon beneath. Reek is likely to be a defensive identity, as becoming Reek and rejecting Theon makes the pain stop.
