While the likes of J.D, Elliott, and Turk are based on real people Bill Lawrence has known in real life, and a lot of the medical side of the stories have come from them, the creator of the show has drawn on his own experiences for writing as well.

The specific instance he talks to Zach and Donald about is part of My Best Friend's Mistake, the topic of the podcast. With Turk spending more and more time with Carla and his new surgery buddies, and less time with him, J.D blurts out that he misses his friend so much it hurts sometimes.

This was something Bill actually said to his fraternity brothers in college about his long distance girlfriend at the time. Naturally, they never let him live it down, and it went into the show as one of the funniest moments of the early episodes. It's a very J.D thing to say, made even funnier by the fact that just seconds before he told himself not to sound like a girl for once.
