Game of Thrones is no stranger to an ambiguous title, and The Last Heir could mean literally anything and refer to any position, honour, house, or title in the world. Some of the best guesses would be Tommen, and more particularly his potential death. If he dies, who does the kingdom fall to?
Theres also the likes of Tyrion possibly being the last heir to the late Tywin and Casterly Rock. Then theres a potentially resurrected Jon Snow who could turn out to be a true Stark and inherit Winterfell if he can take it by force. And there is of course the rest of the Stark family.
