Franks coma-induced hallucinations could each be their own WTF moment if were all being honest. From the confederate soldier vision to the attack in the woods, they are all pretty messed up sequences. But this one takes the cake. The appearance of Zoe Barnes is certainly a startling moment, especially since she looks quite different than she did when she was alive - she now seems to share a passing resemblance to Claire, in fact. The added pleasure of seeing Russo back just ramps up the weirdness and intensity of the scene. The visions continue to show a side of Frank we rarely see. Hes weak, hes vulnerable, and hes overcome by his insecurities. As Frank is overpowered by the ones he killed, he gets a brief reprieve when his vision flashes to himself and Claire together in the Oval Office. As it happens, that vision changes a lot for Frank.
