After Ben's Leukemia went into remission, he went on a soul searching tour across the globe, failing to see a single doctor to check up on his condition. On his return to Sacred Heart after two years he was ordered to get tested by Dr. Cox who put JD in charge of him.

Despite JD's protests of being busy, and being worried about his other patient Mr. Taylor, Cox stood firm, telling his protégé that his patient wouldn't die in the next 30 minutes. However, JD told Cox on his return to the hospital that he went into cardiac arrest, and there was nothing they could do.

The episode is designed so that the audience thinks it is Mr. Taylor that has passed. Ben is still hanging around with Perry, and he wasn't the one they were actually worried about. But watching it back, Cox is the only one to interact with his friend, and Ted, Dr. Kelso, Carla, and Turk are seen dressing for a funeral. No one could put this together however, that it was actually Ben that died.

Dr. Cox thinks he's at his son's birthday party, but him turning around to see that Ben is gone is one of the most heart wrenching moments, not only in Scrubs, but in sitcom history. This is supposed to be a comedy show, but it's impossible to watch the reveal of Ben's death without a tear in the eye. This was truly excellent story telling, finishing with a gut punch unlike any other.
