A patient under his care might not be too excited to hear Dr. Death is coming, but the name definitely advertises a tough guy approaching. "Dr. Death" implies years of training and hard work at some tough-guy medical school, having worked his way up from a Bachelors' degree in Death and a Master's degree in suplexes that drop you right on your head, before finally reaching the promised land.

That nickname certainly helps, because "Steve Williams" is pretty bland by itself.

Steve Williams sounds like a guy who gets knocked out by Bart Gunn. "Dr. Death" sounds like a man who terrorizes Japan as part of The Miracle Violence Connection and goes a decade without being pinned.
