1. Perro Aguayo Jr.
Perhaps the most recent neck breaking injury on the list is also the most heart-breaking, as it is the only one that has led to a death in the ring.
Perro Aguayo Jr. had been hyped as the next big thing to come out of Mexico for a number of years and seemed to be well on his way to eventually ending up in WWE at some point in the future. Unfortunately a tag match featuring Aguayo, Rey Misterio Jr. Manik and Xtreme Tiger in Tijuana, Mexico ended up costing him his life.
Towards the end of the encounter, Aguayo was sent to the outside with a headscissors by Misterio. He seemed somewhat groggy as he got back into the ring and he was limp by the time he had been dropkicked into the 619 position.
To his credit, Rey noticed that something was wrong immediately and made sure to miss with his 619, but the incompetence that followed could not be excused.
Aguayo was shaken multiple times by Konnan in an attempt to revive him, with the show medic apparently working on two other wrestlers backstage.
As it turned out, Aguayo had suffered three fractured vertebrae, most likely caused by impact with the ropes following Misterio's dropkick.
He died in the ring of cardiac arrest caused by a cervical stroke in one of the biggest wrestling tragedies in recent history.
