WWE had reactivated the United States Championship in 2003, and the company organised a multi-man tournament on SmackDown to crown the new champion. Eddie Guererro won the title with a victory over Chris Benoit in the finale. The likes of Big Show, Booker T, and John Cena held the title before Carlito Caribbean Cool defeated 'The Champ' to win his first US championship.

The Puerto Rican wrestler was making his debut on the blue brand, and by knocking out 'The Face That Runs The Place' with a steel chain, he seized the mid-card title. Although he lost the title to Cena after just 42 days, Carlito later defeated Big Show in a match at Judgement Day. The backstage officials moved him to RAW in 2005, where he overcame a challenge from Shelton Benjamin to capture the Intercontinental Championship.

Despite winning the two mid-card titles of the company on his brand debut, he could never reach the number one position in the industry.
