4. Breaking Bob Holly's Neck
Break someone's neck, and it's going to be controversial, no matter what the circumstances. It doesn't help when that person is someone many believe you have something of a feud with. Bob Holly had a bit of a reputation as a guy who worked stiffed, so after a 2002 bout in which he suffered a broken neck (followed by a 13 month layoff) as a result of a botched power bomb by Lesnar, rumours swirled that it was Lesnar sending a message. Which was total garbage, as confirmed by both men after the fact. Yet the controversy raged for months. In reality, Lesnar simply couldn't get Holly up into the proper position, and he took a bad bump, pretty much landing on his head. After healing up, he would actually go on to work an angle involving the injury with Lesnar and challenging for Lesnar's title.
