One of the more recent examples of a champion losing his title and not getting a shot at regaining it, Triple H very notably won the WWE World Championship at Royal Rumble 2016 and lost it at that year’s WrestleMania to Roman Reigns.

However, Helmsley did not ever challenge Reigns for his title again, essentially allowing the new champ to proceed unimpeded on his path... which ended a few months later when Seth Rollins defeated him.

Triple H found his way back into the Mania main event conversation months later when he began a feud with Rollins (presumably over who could use the Pedigree the best), but he has not attempted to regain the championship since.
