4. Brandon Lee's Death - The Crow
Brandon Lee's death is one of the most talked about moments in the grim history of on-set tragedies, particularly after the death of his father Bruce Lee at just 32-years-old.
On the set of The Crow in 1993, Lee was filming an action sequence that involved using a gun loaded with blanks. Catastrophically, the .44 Magnum contained a dummy cartridge left over from a previous scene that required close-ups (though it didn't contain powder or a primer).
Unfortunately, the crew who made the dummies themselves left the rear primer and one dummy bullet was able to fire into the barrel, which went unnoticed. When the blanks were added with live powder and primer, the lodged dummy round was shot out accidentally, striking Lee in the abdomen and fatally injuring him. Despite six hours of surgery, he died at the age of just 28.
