Alternate timelines are a funny thing. In a way, they're a handy way for the writers to let out some of their bloodlust, as the various members of the SGC teams have died many, many times across the years. Very few are immune to this, including, in this case, the unfortunate Colonel Samantha Carter.

Sheppard is involved in an accident through the gate, one that sends him thousands of years into the future. This comes at a critical time in the Pegasus galaxy, as the wraith Michael Kenmore was conquering much of the known planets. Without Sheppard, the Atlantis expedition lost vital intel, indirectly resulting in the deaths of billions of humans throughout hundreds of worlds.

In this timeline, the International Oversight Advisory ordered the Atlantis team home, but Colonel Carter ignored them. Instead, she worked tirelessly to bring a Daedalus-class ship, the Phoenix, up to fighting strength. She inflicted considerable damage on Michael's fleet, in a series of guerilla hit and run attacks. Her luck finally ran out with Michael fed misinformation to her, resulting in an ambush. She evacuated her crew, then set a collision course.

Carter had survived hundreds of encounters with Gou'uld, Replicators, and even the Ori. In the end, she died destroying several hive ships. The haunted looks on her eyes suggested that even knowing her death would inflict significant damage to Michael's forces, it was still death in the end.
