5. Lindsey Perez - Saw IV
Agent Lindsey Perez (Athena Karkanis) first shows up in Saw IV as one of the cops investigating Jigsaw's continuing rampage, but she sadly appears to meet her end when attending the aftermath of Jiggy's latest trap.
There Perez finds one of Jigsaw's iconic Billy puppets, which promptly explodes, firing shrapnel into her face and taking her out of commission for the rest of the movie.
In Saw V, we learn that Perez succumbed to her injuries, and that seems to be the end of it. But then, in a genuinely surprising twist, Saw VI reveals that she's in fact still alive, and conspired with colleague Agent Erickson (Mark Rolston) to stage her demise for her own protection.
This ends up being quite the surprise to the new Jigsaw, Detective Hoffman (Costas Mandylor), but sadly Perez actually dies for real near the end of the movie after Hoffman stabs her repeatedly in an attempt to cover his own tracks.
