2. Tony Blames Himself - Spider-Man: Homecoming
You really have to feel for Tony Stark: every time he develops a close relationship with someone, they end up in grave danger. His parents were killed by The Winter Soldier, he saw Happy Hogan almost murdered by an Extremis attack, Pepper Potts was infected with the same disease, Rhodey was paralysed in Civil War and then Peter Parker ended up being killed at the end of Infinity War. If he had a dog, it would have been run over by now.
And for anyone who was doubting that Parker would end up going the same tragic way, Tony himself predicted it when he tried to stop Spider-Man from becoming an Avenger. He literally tells him that "if you died I feel like that’s on me. I don’t need that on my conscience.”
Anyone watching Infinity War after Peter's death will know that he definitely called it. At that point Tony looks a broken man, having encouraged Parker to be an Avenger (after initial reluctance) and it's definitely going to be his guilt that drives him in Avengers 4.
