Perhaps one of the most famous on set fatalities is known from the movie The Crow, released in 1994 as a dark superhero film starring Bruce Lee's son Brandon as the main role. Many have claimed his interpretation of the villain has inspired future antagonists in cinema history, including Heath Ledger's The Joker, but nobody wishes to undertake the disaster that happened whilst filming, causing many scenes to be rewritten and production to be extended before release.

From the very start of shooting day one, the production crew received a mysterious voicemail that warned them not to make the movie. On this very day, an electrician for the film caught fire after hitting the electric wires above him and was hospitalised. After this, a hurricane spread through set and word began to spread that the movie was really cursed.

It is believed from past stories that a gun used on set was purposefully swapped bullets moments before calling action, causing Brandon to be shot by a real bullet instead of fake. In fact, this was not the case. Nobody realised that a dummy round had become lodged in the barrel, causing the speed of the fake to fire out at the same speed as a real round after blank rounds were added to simulate gun fire.

Lee was hit and later died in hospital.
