In Smile, a psychiatrist called Rose Cotter discovers she is being pursued by a demon. Within a matter of days, everyone who encounters this nebulous being is driven irreversibly insane, compelling them to take their own life.

And all this malevolent force does to drive people mad is create illusions that involve smiling.

At first, this strategy may not sound intimidating. In fact, it sounds pretty dumb. But the Smile Entity knows precisely when to appear to leave Rose utterly rattled. Striking when she's at her most vulnerable, the calm and collected doctor turns into a nervous wreck almost overnight. By taking the form of her colleagues, clients, and family, Rose pushes everyone away, making it easier for the Entity to toy with her.

However, the reason the Smile Entity is so effective at breaking its victims' minds is because it offers them false hope. During the climax, The Entity deliberately allows Rose to believe she has beaten it for good. When the Entity returns to haunt her, Rose realises she will never be free of it, urging her to kill herself.
