6. Costas Mandylor Didn't Know If Hoffman Died In Saw VI

At the end of Saw VI, John Kramer's wife, Jill, attaches the Reverse Bear Trap to Hoffman's head before leaving him to die. Luckily, Hoffman manages to rip the device off at the last second. Even though the last frame shows Hoffman surviving the trap (missing one cheek), the actor portraying him, Costas Mandylor, still wasn't sure about the character's fate at the time of filming. To keep the conclusion a secret, Mandylor filmed multiple endings, including one where Hoffman perished.

Despite the fact the writers have made it abundantly clear they will do all they can to protect the movies' twists endings, one would assume the main actors would be clued in. But as Mandylor proved, that is not always the case.
