4. Wanda's Accident In Lagos
By the conclusion of Avengers Endgame, Wanda Maximoff had grown into one of the most powerful characters in the entire MCU. She has had the powers the mind stone granted her, but she was not always able to control them.
Wanda's journey started on the opposite side of the battlefield to Stark and co., but during Age of Ultron she switched sides to officially become an Avenger. By the time she followed Crossbones to Lagos with Cap, Nat, and Sam, she had a tighter grasp on her powers, but still far from full control.
Showing up just in time to save Captain America from Rumlow's explosives, Wanda compounded the explosion and sent the villain into the sky. Only she accidentally took this concentrated blast and blew up part of a building full of innocent people.
For General Ross and the US Government, this was the straw that broke the camel's back, convincing law makers across the planet that the Avengers were unstable and no longer trusted with the absolute power and authority they had been granted before. Though this was not Wanda's intention, if she wasn't in full control of her powers, she shouldn't have been risked in the field.
Perhaps Steve Rogers should take a share of the blame for this, as he would have known that there was always a risk of something like this happening.
