In Vampire: The Masquerade - Bloodlines the modern-day Los Angeles setting is used to make the vampires appear more out of place. It would be hard to understand the importance of the titular "masquerade" if the game took place in a traditional fantasy setting where a vampire wouldn't be out of the ordinary.

Maintaining this masquerade is the biggest way the setting plays a part in the gameplay, showing off your vampire powers to humans will result in you losing Masquerade points, losing five of these will end the game, so yeah, you have an incentive to not go around telling everybody your secret, even if it is a really cool secret.

Again, in a traditional fantasy setting nobody would be surprised to see a vampire so this system wouldn't work as well as it does in a modern setting.

Vampire: The Masquerade - Bloodlines is just a really interesting game that happens to take place in the present. Walking among clueless humans and maintaining your humanity or playing as a Malkavian if you really want a weird experience is a lot of fun, it's worth picking this game up before Vampire: The Masquerade - Bloodlines 2 comes out.
