There are a number of ways this can be done, such as incessantly rolling "need" even when they don't need an item, to slyly changing loot parameters mid-battle, such that they can quickly hoover up all the loot once the fight is over. Patches have attempted to remedy the ability to exploit the game's system - and have succeeded to an extent - but it also often ends up denying items to those who have genuinely rolled need. Still, back in the day, I'm sure we've all done it at some point, especially if we're playing with more capable, higher-level friends; it's a game with a fairly obnoxious user-base anyway, so one won't feel too guilty about skimming a little off the top.
