Like many deep fantasy worlds, Obsidian Entertainment's Avowed is brimming with distinct factions vying for power. Some are altruistic, whereas others are not, and sitting far, far, far on the "not" side of that spectrum is the fanatical Steel Garrote. These ruthless Paladins, led by the fearsome Inquisitor Lödwyn, aim to hunt down anyone who violates a contract as it defies their chosen deity, Woedica. Overall, they aren't the most forgiving of people.
The option to side with these fanatics in the "Siege of Paradis" quest will, ordinarily, lead to you getting beheaded. However, if you make every choice throughout the game that aligns with Lödwyn's ideals, she will instead welcome you into the ranks of the Garrote, recruiting you in service of Woedica, The Exiled Queen.
Given how siding with these tyrants requires you to endorse and perform many heinous deeds and causes your companions to abandon you, it's unsurprising that only 1% (via True Achievements) of Avowed's player base have actually joined their ranks.
