Speaking of ending twists that went on to dominate their respective games for quite some time, Fallout 3's original climax had you needing to enter a room filled with radiation... that would inevitably kill you.

"Oh, but there are all these Radaway radiation-blockers. I can just have some of them an-" No. Now come on, get in the room. Get in there.
Such was the state of post-Fallout 3 memery, leading to Bethesda actually patching in a completely different ending that not only acknowledged the in-game context of how to survive radiation, but let you continue exploring once the credits had rolled.
Credit to Bethesda - it only took them six months to unf*ck themselves on this one, which is more than you can say about Fallout 4's "I'm in a nuclear wasteland and have zero questions to ask" protagonist.
