7. Giving Up The Mole Rat Cure - Fallout 4
Fallout 4 doesn't feature a proper disease system like the Elder Scrolls series, but it does secretly come with one permanent affliction that can impact the player without them knowing.
The so-called Mole Rat Disease is a -10 HP debuff that you can obtain without the game informing you of it when you are bitten by one of the mole rats infesting the hidden research facility built into the walls of Vault 81.
The infection can also become permanent if you make a hasty decision without checking your stats.
The molerats and their abandoned dwelling are part of a side quest in which you look for a cure to the disease to save a young Vault Dweller named Austin. After you venture into the facility, it turns out there is only one dose of the vaccine, so if you have become infected yourself, giving it to Austin can leave you with a permanent debuff to your health.
The choice quickly becomes regrettable when that -10 loss of health starts allowing stronger opponents to one-tap you, so you best keep the cure for yourself or keep track of your debuffs and reload the game when it turns out you need to kill a child to keep your character healthy.
