Shinki Mikami's awesome little horror title remains one of the most overlooked on all current hardware, but it's a game with a penchant for brutal difficulty through ammo scarcity and enemies that are far more nimble than your character, Sebastian Castellanos. The 'You Earned It' achievement is quite possibly the most instantly "NOPE!"-sounding of the lot on this list; beat the entire game in AKUMU mode, meaning any one hit will kill you. Yup, it's that 'simple'. Avoid the bag-headed chainsaw men, survive against the many screen-filling bosses who already posed quite the threat, and - especially in the latter levels - somehow stay in one piece when the level itself is deforming around you.
