While being sent to locate Shabani in the Angola-Zaire border region, Snake uncovers a medical centre covered in gore and convulsing corpses in body bags. As he goes through a blood-stained curtain, he comes across a CFA soldier with lesions on his lungs, and an earphone sewn into his throat.

Snake investigates further, and finds Shabani, the child soldier, strapped to a gurney. Snake breaks Shabani free before spotting Skull Face for the first time in the game. Skull Face goes to another prone soldier and speaks Afrikaans before blowing the solider’s head off.

Shabani begins to wince, and Skull Face commands Tretij Rebenok and the Man on Fire to burn the place down, starting a boss battle for the player.

The limited dialogue and shaky camera movements add to the scene’s chilling atmosphere. Skull Face remains elusive, as he scarpers during the ensuing boss battle with the Man on Fire.

Later, it is revealed that Skull Face plans to eradicate English with the Vocal Cord Parasite, and it seems as if the earphone in the CFA’s soldier’s throat was repeating his infected language back to him as an indescribable method of torture.

The majority of cutscenes in Metal Gear Solid V are darker than in previous instalments of the franchise, but this one remains one of the most twisted and chilling.
