The Secret Cow Level is one of the most beloved and well-known easter eggs in Diablo II, and for a good reason, since learning how to access this special location is one of the most effective ways of levelling up your character and farming items from the swarms of bovine enemies inside.

However, keep in mind that you can only enter it once per campaign.

To unlock the cow level, you need to grab Wirt’s Leg in the Rogue Encampment and then transmute it with a Tome of Town Portal inside the Horadric Cube. This will cause a red portal to appear on the plaza of Tristram. Inside this teleport is a realm filled with halberd-wielding cows that swarm you in giant hordes while making the most unenthusiastic “moo” noises you can imagine.

In the original version of Diablo 2, the portal to the cow dimension could only be accessed once. After your character defeated the Cow King at the end of the level, the game would permanently lock you out of entering the level again.

The later patches made this restriction less harsh by locking the level behind what difficulty you beat the game on and allowing you to re-enter the cow level on a new game set to the same difficulty. This still means, however, that you should make the most of your visit to the cow dimension.
