3. Being Crucified And Castrated In Outlast 2
Outlast 2 is so brutal that it could have taken up every entry on this list and then some, but it's both the super-serious crucifixion sequence and the game over animation that sees your character's junk being mutilated by a giant pick axe that best reflect the endless horrors the game has to offer.
The crucifixion set-piece is absolutely harrowing, coming out of nowhere at the end of a chase sequence. The player is rendered helpless as they watch their character being nailed to a cross in an extended piece of gameplay, with the title making you feel every single hammer blow as you're mangled and suspended in a field and left for dead.
On the other hand the penis mutilation stuff is shocking because it's just a regular death animation, indicative of how even Outlast 2's most basic game-over states want to punish you in the most over the top ways possible. You might think that it would lose its impact after you see it over and over again, but it never once stops making you cringe.
