Kurt Cobain spent days setting up and arranging the college that would be the back cover of In Utero, on his dining room floor. Consisting of plastic fetuses, a headless anatomy mode, a model turtle and muli-colored flowers. According to Cobain, the collage covered themes from the album such as birth, death, and women.

The shot was taken by photographer Charles Patterson, who was reportedly hovering over the collage as Kurt urged Patterson to take the picture before the flowers dried. Kurt's creative collage based on the human anatomy would later affect In Utero's sales in some stores.
