The original title of the song Heart-Shaped Box was Heart-Shaped Coffin. The title was based on a collection of heart shaped boxes Cobain and Courtney Love began collecting when they were together.

Outside of that, a deeper meaning is unclear. Like with In Utero, Kurt was persuaded to change the title as it came off as too dark, and would never receive any play time from ay mainstream radio stations.

Although the song would never reach the success of Nevermind's first single, Smells Like Teen Spirit, Heart-Shaped Box received a lot of playtime on MTV. The song is still regarded as one of their best.
