4. Black Sabbath - Milwaukee (1980)

http://www.youtube.com/watch?v=3SZM-jEpbTs Hot on the heels of Ozzy's departure from the band and the inauguration of The Dio Years (Named after the late angel-voiced badass Ronnie James Dio), the band were to play a gig in Milwaukee, however perhaps due to the Sabbathian diehards assuming they would be lifelong Ozzy Osbourne-worshippers, Dio's words to the crowd following a blackout in the arena did nothing to dissuade their anger at the band arriving late. During said blackout, a hurled bottle collided with bassist Geezer Butler, covering him in glass and then his own blood, requiring he be taken off. As the lights came back up, the band were nowhere to be found, being they were all taking care of their fallen comrade. Geezer recalls the entire incident as a "big misunderstanding" and a lack of communication between promoters exacerbated proceedings somewhat. That's if you can call the above photo an exacerbation of course, being that the vast majority immediately uprooted their chairs and converted the stage into a makeshift steely-recreation of Mount Doom.
