Joffrey would command others to do his evil bidding, whereas Ramsay is judge, jury and executioner throughout his time on the show. He started out with the psychological obliteration of Theon Greyjoy, including the removal of a certain important piece, before tauntingly wolfing down a sausage in front of him. If that doesn't scream evil, nothing will. But when you think he can't get any worse BOOM, he does. There's the Sansa rape scene which was thoroughly distressing to watch, and the flaying of an old woman for aiding Sansa. That's just two examples, but the Ramsay highlight reel would be a grim, grim piece of film to endure.
