The first episode of Series 4 permanently paired Donna Noble with the man who rescued/ruined her wedding day.

Donna and the Doctor join forces to investigate Adipose, a diet pill manufacturer with a dark secret. Instead of headaches or blurry vision, the side effects of this medication involve users' fat being turned into tiny, blubbery aliens.

These Adipose children are being born on Earth because their species' original nursery planet was stolen by the Daleks. As a result, the Adiposian first family employed a space nanny called Matron Cofelia, to find them a new breeding ground.

Of course she chose Earth. They always choose Earth.

While the good guys manage to stop full conversions of the people who took the pills, several thousand Adipose children were created. Their parents come to pick them up, killing Cofelia to cover their tracks.

So while Cofelia doesn't get away with her crimes, the Adiposian First Family ends up getting what they want: they have a batch of new younglings with nothing to tie them to their illegal breeding scheme.
