7. Dr Blake Downs (Childrens Hospital)

Batman's greatest nemesis is a clown, and many of his other enemies are doctors of some variety, so why not combine the two? Downs' permanent decoration in both face paint and blood-stained scrubs would make for a fantastic supervillain aesthetic, and Childrens Hospital is littered with references to morally questionable or even downright illegal actions undertaken by Brazil's finest clown surgeon.
He could equally be played as the zany, comedy character he is on the surface to serve some of Batman's lighter stories, or be used as a terrifying psychopath given his propensity for occasionally accidentally killing children, a fact which he seemingly refuses to ever let upset him.
Like many comic book characters, Downs has been killed and subsequently resurrected, so it shouldn't take too much adjustment to refit this larger-than-life character into yet another terror of Gotham's citizens.
He has the look, he has the tools and he's egotistical and unscrupulous enough to have the motivation, too. He's even partially designed to resemble serial killer John Wayne Gacy, a sinister implication that adds just that extra bit of darkness and realism that's so often sought for in Batman stories in the 21st century.
