Here we have an example of a title which described a very specific arc, but once that arc ended, nothing could be done to fix the name. The characters did indeed break out of prison at the end of the first season, and then spent the second season as fugitives. The writers apparently realised this flaw in their grand plan and proceeded to intermittently send other characters to prison, so that they required breaking out and the title was less of an inconvenience. Strangely enough, Australian television actually did rename the second series Prison Break: On the Run, which is a neat way of sidestepping the issue. With the revival of the anthology series on television at the moment, perhaps we'll see more shows which fulfil their potential as miniseries rather than stretching a premise far beyond its limits.
