2. Ben Sullivan's Missing Camera

As noted, Ben Sullivan was played by Brendan Fraser, and suffered one of the most cruel and surprising fates in the entire show. He originally appeared in two season one episodes, where he was diagnosed with leukemia. An avid photographer, he was best friends with Cox and took his camera everywhere.

In the season three episode "My Screwup", Ben is admitted to hospital and Cox gets annoyed with J.D after a patient dies on his watch. Ben spends the episode trying to coach Cox through his anger, only for the last moments of the episode to reveal that Ben is the one who has died.

It's a shock twist, but if you're playing close enough attention you can kind of see it coming. During his short stay on the show, Ben was always shown with his camera, until after he died. For the rest of "My Screwup", Ben is camera-less, and it's only after realising this that Cox accepts his friend has died.

It's easy to miss, but brilliantly done.
